Job Description

Yoakum Community Hospital, Yoakum, TX has a full-time, part-time or PRN position available for a Licensed Clinical Social Worker (LCSW) at Yoakum Community Hospital in Yoakum, TX.  This is a great position and package with paid time off and benefit options for regular and part-time positions. The Social Worker, reporting to the Program Director, is responsible for providing high quality patient care, displaying good skills in assessment, psychodiagnosis, treatment planning, group treatment, individual treatment, family treatment and discharge planning.

Duties and Responsibilities:
●       Assess and reassess the needs of the patient, perform proper documentation of assessments.
●       Able to communicate the needs of patients and always look for ways to improve the patient experience.
●       Completes psychosocial evaluations on assigned patients in a timely fashion. Displays good interviewing, diagnostic, and report writing skills. Conducts therapies in accordance with patient diagnosis and needed. Provides efficient discharge planning throughout treatment process. Completes progress notes and assessments according to program procedures. Completes individualized treatment plans for each patient.
●       An active member of the interdisciplinary team. Effectively participations in the referral development process.
●       Conducts individual psychotherapy, family therapy, and group therapy sessions in accordance with state/regulatory standards, displaying good theoretical knowledge of therapy principles and good treatment technique.
●       Engages in timely, efficient discharge planning. Works in close conjunction with patient, family, nursing staff, and attending physician. Shows good knowledge of referral/placement process and available community resources.
●       Provides ongoing education to patient and family, as needed. Conducts educational group sessions, displaying theoretical knowledge of subject matter and effective communication skills in presenting material.
●       Communicates effectively with families and agencies, conveying a positive, professional image. Provides resources and support for patient before, during and after treatment of patient.  Actively involved in measuring patient performance.
●       Communicates results and trends to Director on ongoing basis in regards to concerns, improvements, changes, etc. Identifies areas needing improvement and utilizes the facility performance improvement process. actively participates in department processes as required. Brings to Director’s attention significant issues related to program functioning.
●       Performs inpatient assessment duties as directed by the Psychiatrist on a monthly rotating weekend schedule.
●       Build relationships to promote a collaborative environment
●       Be accountable for your performance

Job Requirements

  • Master’s degree required AND

  • Experience in hospital or office and/or training; OR

  • Equivalent combination of education and experience

  • Current license in the State of Texas

  • Current certification to perform Clinical Supervision for program LMSW(s)

Qualification Requirements:

  • Handle a high degree of responsibility for confidential matters

  • Excellent communications skills required with a high degree of organizational skills

  • Must be computer literate

  • Ability to communicate professionally and effectively with all whom contact is made

  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations

  • Ability to perform duties independently with little or no backup. Ability to multi-task

  • Ability to define/solve problems, define complex problems, deal with a variety of concrete variables in situations where only limited standardization exists, collect data, establish facts, and draw valid conclusions

  • Ability to interpret an extensive variety of instructions furnished in written, oral, diagram, or schedule form, and deal with several abstract and concrete variables

  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations
